Community First Services and Supports (CFSS)

What Is CFSS?

CFSS will replace Personal Care Assistance (PCA) and Consumer Support Grant (CSG). Community First Services and Supports (CFSS) is a Minnesota healthcare program that provides flexible options to meet individuals’ unique needs, promoting greater independence in their homes and communities.

CFSS will cover the same main services as PCA:

• Activities of daily living, including eating, bathing, grooming, and transferring
• Health-related tasks
• Instrumental activities of daily living, such as shopping, cooking, and laundry
• Monitoring and redirecting behavior

MORE OPTIONS AND SUPPORTS WITH CFSS

• Individuals who wish to continue receiving support from a provider agency can do so through the CFSS Model.
• The CFSS budget model gives people greater choice and control as the employer of their support workers!

CFSS is a flexible and person-centered program that gives clients more control over their care. Through CFSS, individuals can choose and manage the services and support they need. Our CFSS offerings include:

• Support planning and coordination.
• Assistance with setting and achieving personal goals.
• Skill development for greater independence.
• Access to equipment and technology to support daily living.
• Guidance in hiring and managing care workers.

CFSS FAQs

Am I Eligible?
The eligibility criteria for CFSS will remain consistent with those for PCA. If you qualify for PCA services, you will also qualify for CFSS unless there is a change in your condition.

• 01. Reside in the community rather than in a facility.
• 02. Be capable of directing their care or have a representative who can do so on their behalf.
• 03. Be enrolled in one of Minnesota’s healthcare programs: Medical Assistance (MA), an MA waiver, the Alternative Care program, or certain individuals on MN Care.
• 04. Undergo an assessment to determine eligibility.
• 05. Not be receiving the Family Support Grant.
