245D Licensed Services are based on the new Minnesota Statute, which went into effect starting January 1, 2014. As a licensed provider, Midwest Quality Home Care, Inc. is required to follow Chapter 245D policies and procedures for our Home and Community-Based Services. These are licensed services that need additional regulations and oversight requirements that are intended to improve the quality of health and promote the independence of the clients receiving these services.
Community First Services and Supports (CFSS)
> What is CFSS?
CFSS will replace Personal Care Assistance (PCA) and Consumer Support Grant (CSG). Community First Services and Supports (CFSS) is a Minnesota healthcare program that provides flexible options to meet individuals’ unique needs, promoting greater independence in their homes and communities.
> CFSS will cover the same main services as PCA:
- Activities of daily living, including eating, bathing, grooming, and transferring
- Health-related tasks
- Instrumental activities of daily living, such as shopping, cooking, and laundry
- Monitoring and redirecting behavior

- Individuals who wish to continue receiving support from a provider agency can do so through the CFSS Model.
- The CFSS budget model gives people greater choice and control as the employer of their support workers!
CFSS is a flexible and person-centered program that gives clients more control over their care. Through CFSS, individuals can choose and manage the services and support they need. Our CFSS offerings include:

What will current clients need to do to transfer services to CFSS? What are consultation services?
What are the two delivery model options?
CFSS will offer two service delivery models: the agency provider model and the budget model. Midwest Quality Home Care, Inc. will provide both models, so we can deliver your CFSS home care services regardless of the model you choose.
In both models, you or your responsible party:
- Directs all care;
- Has a say in selecting your worker, who can be your spouse or parent if the person needing care is a minor;
- Writes the plan with assistance from the consultation services provider, if desired; and
- May purchase goods.
In the agency provider model, you will choose a provider to act as the caregiver’s employer. DHS will use PCA agencies, including Midwest Quality Home Care, Inc., to deliver these services.
If you were already receiving PCA services, you can retain your current, trusted caregiver. If you do not have a caregiver, Midwest Quality Home Care, Inc. will make a reasonable effort to fulfill your request for a preferred support worker.
In the budget model, you will employ your support workers and have a budget instead of service units. You will be responsible for recruiting, hiring, training, and supervising your support workers. You will also select a financial management services (FMS) provider, including Midwest Quality Home Care, Inc., to help ensure compliance with applicable laws.
Will Midwest Quality Home Care, Inc. offer clients both delivery model options?
Midwest Quality Home Care, Inc. will provide both the agency model and the budget model, allowing us to serve both current and future clients under CFSS. With experience as a PCA Choice provider and an FMS provider, Midwest Quality Home Care, Inc. is well-equipped to continue serving current clients as they transition to CFSS services and select the model that best meets their needs.
What are some of the benefits of CFSS?
People who use CFSS will have more options and supports:
- Unlike PCA, CFSS allows a person’s spouse or the parent of a minor to serve as their support worker.
- Individuals may choose to purchase goods that help promote their independence.
- The state offers a budget that the provider agency or the individual can use to train workers to meet the person’s specific needs.
- The consultation services provider will offer education and support in creating the person’s plan.
When will the transition to CFSS start?
The transition for all current PCA and CSG service users to CFSS is expected to take 15 months, meaning your transition to CFSS may occur a year or more after the program starts on Oct. 1.
> Am I Eligible?
The eligibility criteria for CFSS will remain consistent with those for PCA. If you qualify for PCA services, you will also qualify for CFSS unless there is a change in your condition.
01. Reside in the community rather than in a facility.
02. Be capable of directing their care or have a representative who can do so on their behalf.
03. Be enrolled in one of Minnesota’s healthcare programs: Medical Assistance (MA), an MA waiver, the Alternative Care program, or certain individuals on MN Care.
04. Undergo an assessment to determine eligibility.
05. Not be receiving the Family Support Grant.
